簡(jiǎn)要規(guī)格描述或項(xiàng)目基本概況介紹、用途：為老人提供專業(yè)的居家生活護(hù)理、助浴服務(wù)、助潔服務(wù)、洗滌服務(wù)、助行服務(wù)、代辦服務(wù)、輔助康復(fù)服務(wù)、相談服務(wù)、助醫(yī)服務(wù)，提高老人的生活質(zhì)量，讓老人舒適、整潔、方便、健康、幸福，讓老人在熟悉的居家養(yǎng)老環(huán)璄中頤養(yǎng)天年。（具體要求詳見(jiàn)招標(biāo)文件—第三章 采購(gòu)需求書）            

Brief specification description or basic overview of the project: Provide professional home life care, bathing assistance services, cleaning assistance services, washing assistance services, mobility assistance services, agency services, auxiliary rehabilitation services, consultation services, and medical assistance services for the elderly to improve their quality of life, making them comfortable, clean, convenient, healthy and happy, and allowing them to enjoy their old age in the familiar home-based elderly care environment 璄. For specific requirements, please refer to Chapter Three of the bidding Documents - Procurement Requirements.            

合同履約期限：本項(xiàng)目服務(wù)期限1年，具體以合同簽訂為準(zhǔn)

二、申請(qǐng)人的資格要求
2. Qualification Requirements for Bidder

(a)滿足《中華人民共和國(guó)政府采購(gòu)法》第二十二條規(guī)定；

(a)Meet the provisions of Article 22 of the "Government Procurement Law of the People's Republic of China";

(b)落實(shí)政府采購(gòu)政策需滿足的資格要求：（1）項(xiàng)目執(zhí)行《政府采購(gòu)促進(jìn)中小企業(yè)發(fā)展管理辦法》的通知（財(cái)庫(kù)〔2020〕46號(hào)）及《關(guān)于進(jìn)一步加大政府采購(gòu)支持中小企業(yè)力度》的通知（財(cái)庫(kù)〔2022〕19號(hào)）；（2）落實(shí)預(yù)留份額措施，提高小微企業(yè)在政府采購(gòu)中的份額，扶持小微企業(yè)政策：本項(xiàng)目專門面向小微企業(yè)采購(gòu)，評(píng)審時(shí)小微企業(yè)產(chǎn)品均不執(zhí)行價(jià)格折扣優(yōu)惠；（3）扶持殘疾人福利性單位，并將其視同小微型企業(yè)；（3）扶持監(jiān)獄企業(yè)、殘疾人福利性單位，并將其視同小微型企業(yè)；（4）優(yōu)先采購(gòu)節(jié)能環(huán)保產(chǎn)品政策：在技術(shù)、服務(wù)等指標(biāo)同等條件下，對(duì)財(cái)政部財(cái)庫(kù)〔2019〕18號(hào)和財(cái)政部財(cái)庫(kù)〔2019〕19號(hào)文公布的節(jié)能環(huán)保產(chǎn)品品目清單中的產(chǎn)品實(shí)行優(yōu)先采購(gòu)；對(duì)節(jié)能產(chǎn)品品目清單中以“★”標(biāo)注的產(chǎn)品，實(shí)行強(qiáng)制采購(gòu)。供應(yīng)商須提供具有國(guó)家確定的認(rèn)證機(jī)構(gòu)出具的、處于有效期之內(nèi)的認(rèn)證證書方能享受優(yōu)先采購(gòu)或強(qiáng)制采購(gòu)政策；（5）購(gòu)買國(guó)貨政策：本項(xiàng)目不接受進(jìn)口產(chǎn)品；（6）按照《中小企業(yè)劃分標(biāo)準(zhǔn)規(guī)定》（工信部聯(lián)企業(yè)[2011]300號(hào)），本項(xiàng)目小微企業(yè)所屬的行業(yè)為其他未列明行業(yè)。        

(b)Qualification requirements to be met to implement government procurement policies: (1) The project implements the "Notice on Promoting the Development of Small and Medium-sized Enterprises through Government Procurement" (CAI Ku [2020] No. 46) and the "Notice on Further Increasing the Support for Small and Medium-sized Enterprises through Government Procurement" (CAI Ku [2022] No. 19). (2) Implement the reserved share measures, increase the share of small and micro enterprises in government procurement, and support policies for small and micro enterprises: This project is specifically targeted at small and micro enterprises for procurement. During the review process, no price discount benefits will be implemented for products from small and micro enterprises. (3) Support welfare units for the disabled and treat them as small and micro enterprises; （For more details, please refer to the Chinese announcement.....）        

(c)本項(xiàng)目的特定資格要求：3、 符合《上海市政府購(gòu)買服務(wù)管理辦法》第六、七條規(guī)定；
4、 本項(xiàng)目?jī)H面向小、微型等各類供應(yīng)商采購(gòu)；
5、 本項(xiàng)目不允許轉(zhuǎn)包。        

(c)Specific qualification requirements for this program: It complies with the provisions of Articles 6 and 7 of the "Measures for the Administration of Government Procurement of Services in Shanghai Municipality". 4. This project is only open for procurement from small and micro suppliers of all types. 5. Subcontracting is not allowed for this project.        

(i)符合《中華人民共和國(guó)政府采購(gòu)法》第二十二條的規(guī)定；

(i)Comply with the provisions of Article 22 of the"Government Procurement Law of the People's Republic of China";

(ii)未被“信用中國(guó)”（www.creditchina.gov.cn）、中國(guó)政府采購(gòu)網(wǎng)（www.ccgp.gov.cn）列入失信被執(zhí)行人、重大稅收違法案件當(dāng)事人名單、政府采購(gòu)嚴(yán)重違法失信行為記錄名單；

(ii)Not included in the list of dishonest persons subject to enforcement, the list of parties to major tax violation cases, or the list of records of serious illegal and dishonestacts in government procurement by "Credit China" (www.creditchina.gov.cn), China Government Procurement Network (www.ccgp.gov.cn) ;
